ABOUT THE YELLOW DOT PROGRAM
The Yellow Dot program is a free service designed to assist first responders in providing immediate, life-saving medical care during the critical first moments following a crash or emergency. By placing a Yellow Dot decal on the driver’s side rear window of your vehicle, you can help alert responders that vital medical information is available in your glove compartment.
How to Use the Yellow Dot Program
The Yellow Dot kit contains:
- A personal information card
- A Yellow Dot decal
Steps to Follow:
- Fill out both sides of the personal information card as completely as possible using pencil. Include a recent photo of yourself.
- Place the completed card in a visible location inside your vehicle’s glove compartment.
- For each person who regularly trav

Steps to Follow:
- Fill out both sides of the personal information card as completely as possible using pencil. Include a recent photo of yourself.
- Place the completed card in a visible location inside your vehicle’s glove compartment.
- For each person who regularly travels in the vehicle, complete a separate card.
- Affix the Yellow Dot decal to the driver’s side rear window of your car to indicate to first responders to check the glove compartment for medical information.
- Remember to update the card yearly or during your annual check-up. If you sell your vehicle, remove the Yellow Dot sticker.

You can also use the Yellow Dot program at home. Simply place a Yellow Dot decal on or near your front door to alert emergency responders. Store a completed card for each household member in a clear plastic bag and place it in a visible location in your freezer.
